We are seeking a dynamic, highly motivated Day Centre Outreach Worker who demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.
The Day Centre Outreach Worker is to provide active engagement of day program participants through individual structured interventions and the development and facilitation of workshops, programs, training and group recreational activities.
This position is a unionized, temporary, full-time position. Full-time positions, Monday-Friday. Contract position until March 31, 2023. Two positions available. The rate of pay is $22.31 per hour.
Key Responsibilities:
- Actively engages participants in the Day Centre and broader community to identify specific issues and provide supports related to:
- Socialization in the community/reducing isolation
- Physical health and wellness
- Harm Reduction
- Meaningful activity
- Self-esteem building
- Community inclusion
- Housing
- Plan and implement recreational activities based on individual social, practical and leisure needs.
- Plans, maintains and integrates a monthly calendar of activities that have a balanced therapeutic approach
- Ensures that Day Centre activities are integrated with, and supportive of other Wesley programs that encompass the population using the Day Centre.
- Respond to client needs for assistance, with information and resources that we can provide, encouraging them to take action for themselves.
- Acts as a resource for former clients assisting them with maintaining housing and community connections by providing resources, encouraging them to continue taking action for themselves.
- Supports and empowers participants in enhancing their health, wellness and community inclusion through the development and facilitation of group and individualized social, recreational, volunteer and peer led activities.
- Supports and empowers participants in enhancing their activities of daily living skills through the development and facilitation of group workshops and information sessions.
- Document, maintain and input regularly in individual client files and all program communication logs & information sheets, relevant/required information.
- Use food preparation and minor cleaning of kitchen and Day Centre duties as a means of engaging clients in meaningful activity and discussions.
- Provide coverage for other employees on shift as required.
- Interact with social, health and community service providers as required.
- Monitor the facilities and surrounding outside areas, including portable shower and washroom areas, to ensure safety, security and community acceptance.
- Maintenance of food supplies and all other program supplies as required.
- Respond to emergencies provides crisis intervention and conflict resolution.
- Participate in unit/staff meetings.

As a condition of employment, the successful candidate is required to submit proof of full COVID-19 vaccination to Human Resources. In the event that successful candidates are unable to get their COVID-19 vaccination, completion of an educational session and weekly antigen testing to provide a negative result will be required.
