The North Shore Health Network provides rural healthcare for communities along the north shore of Lake Huron – including services in Blind River, Thessalon and Richards Landing. The North Shore Health Network sites have been serving their communities faithfully for many years providing emergency, acute, complex continuing care, long term care and community support services.

The Role:

The Geriatric Assessment Nurse through advanced training specializes in the care of older or elderly adults. The Geriatric Assessment Nurse is a member of the Geriatric Mobile team that addresses the physiological, developmental, psychological, socio-economic, cultural and spiritual needs of an aging individual.

Responsibilities:

  • Provides a specialized clinical resource for assessing and treating the elderly (65+ years)
  • Provides expert and targeted assessment and treatment services
  • Enhances linkages with hospital and community services
  • Leads the development, implementation, and monitoring of outcomes of targeted patient centred care plans that include the patient and the family/caregivers in collaboration with the Geriatric Mobile Team members
  • Advocates for seniors to ensure that seniors at risk receive the right care, in the right place, at the right time
  • Promotes a model of best practice in Elder Care and an elderly friendly philosophy of care within the inpatient care units
  • Provides education to family/caregivers on how to provide supportive care in their home to enable discharge back to community and/or as an essential caregiver while hospitalized.
  • Provides education to staff on best practice geriatric care
  • Participates on internal and external committees relevant to the responsibilities of the position and assigned as per the NSHN’s framework for committee representation.
  • Continues to advance knowledge of care of the elderly by creating a professional development learning plan

Qualifications:

  • Registered Nurse in good standing with the College of Nurses of Ontario
  • Completion of education and/or training courses related to care of the geriatric patient/client/resident
  • Completed culturally sensitive care courses and incorporated knowledge into practice
  • Minimum of three years of experience working with frail geriatric patients
  • Knowledge in the utilization and care of all mechanical equipment required for patient care
  • Demonstrated competency in cognitive, psychosocial, and physical assessments of patients
  • Ability to function collaboratively on an interdisciplinary team
  • Ability to meet and interact with people in a pleasant, professional, non-judgement responsible and reassuring manner, both in person and on the telephone
  • Proficient in Microsoft environment (Word, excel and PowerPoint)
  • Clear criminal record
  • Excellent interpersonal and communication skills, both written and verbal;
  • Fluently bilingual (English & French) considered a strong asset.

Wages:

$35.95 to $51.47
