Clinical Nurse Manager

Responsibilities:
- Review 2022 OCP Accreditation report for WAHA with interprofessional team leads.
- Meet with stakeholders (groups/individuals) to identify the current state and understanding of MedRec.
- Utilize quality improvement techniques to draft a medication safety framework and future state for the organization.
- In collaboration with the interprofessional team, develop the Medication Reconciliation policy/accountabilities required to support a robust, safe process (what, who, where, when & how)
- Develop an incremental, evidence-based approach for WAHA’s Medication Reconciliation and Patient Safety initiative (admission BPMH; discharge medication reconciliation; electronic discharge prescription; medication counselling prior to discharge including changes/safety; post discharge follow up calls by clinician).
- Collaborate and support the development of a newly established RPN role— “Care Transitions & Medication Safety”
- Develop and implement an education strategy to engage and support the team in the uptake of medication reconciliation across the organization.
- Prepare an evaluation strategy for the initiative identifying successes, efficiencies, barriers to overcome
- Provide regular weekly updates (verbal) to the team and summary written reports (midterm/final) of key activities of the assignment
Qualifications
- RN or Pharmacist
- Rural or Remote experience preferred
- Acute care experience (with LTC experience preferred)
- Experience with MedRec/BPMH implementation
- Familiar with MediTech/Expanse preferred
- Familiar with Accreditation Canada standards around medication safety
- BSN/Masters in Nursing or Health Administration/BPharm/PharmD
